I was at the Kentucky State Fair this week and saw a WKLO Survey from the week of May 18-25, 1958 on display in the antiques section. (Unfortunately, it didn't win any award--but I would give it an honorary blue ribbon for this website!)
It is similar in format to the June 1958 survey on this website. It has the Paul Cowley logo printed in dark green and the survey results mimeographed on the page. Unfortunately, it was folded in half so only the top 20 songs could be read.
My rough transcription of the survey was as follows (apologies for any errors):

The name of the exhibitor was not visible. However, if the owner happens to read this, would you please send a scan or a copy to John Quincy?

Donnie White was a local artist. One of the first to record rock and roll in the area. He was also a sideman for Pee Wee King. "That's My Doll" made it to #10 on the local charts.
